Stimulating Extracardiac Collaterals via Right Internal Mammary Artery Occlusion: Another Step Into an Undiscovered Country.

چکیده
In their search to find ways to reduce ischemia and possibly prevent death in patients with refractory angina, Stoller and Seiler, in 2014, moved into an undiscovered country and advanced a novel therapeutic strategy involving stimulation of extracardiac collaterals through occlusion of the internal mammary artery. In this issue of Circulation: Cardiovascular Interventions, Stoller and Seiler take another step from temporary internal mammary artery (IMA) occlusion now to report on the effects of permanent percutaneous right internal mammary artery (RIMA) occlusion.

Coronary collateral development in swine after coronary artery occlusion.
We have quantified the development of the coronary collateral circulation in the pig. The collateral circulation was induced to grow by placing an ameroid occluder on the left circumflex coronary artery. Two to 16 weeks after ameroid placement, the coronary collateral circulation was identified after the injection of several colors of a silicone polymer into the coronary arteries and the aorta....
متن کاملEffect of Permanent Right Internal Mammary Artery Closure on Coronary Collateral Function and Myocardial Ischemia
BACKGROUND The objective of this study is to test the effect of permanent right internal mammary artery device closure on coronary collateral function and myocardial ischemia. METHODS AND RESULTS This was a prospective, open-label clinical trial in 50 patients with coronary artery disease. The primary study end point was coronary collateral flow index as obtained during a 1-minute proximal ri...

PURPOSE To present a new approach route for recanalization of a chronically occluded superior mesenteric artery (SMA). TECHNIQUE Percutaneous treatment of an SMA occlusion can be accomplished in some cases via retrograde crossing through collaterals from the celiac artery. From a right common femoral artery (CFA) approach, an 8-F RDC guide catheter is advanced to the origin of the celiac arte...
متن کاملAnterograde flow compromise of a patent left internal mammary artery graft from a proximal subclavian artery stenosis. Myocardial ischemia not driven by the coronary-subclavian steal syndrome mechanism.
A 54-year male with previous triple vessel coronary artery and aorto-bi-femoral bypass graft surgeries complained of crescent angina. Stress induced myocardial ischemia on echocardiography was demonstrated. We performed direct stenting of a saphenous vein graft to the right coronary artery, via right radial approach. Subsequently stenting of a severe left subclavian artery proximal stenosis was...
متن کاملCreation of Anastomoses Between an Extracardiac Artery and the Coronary Circulation
The purpose of this investigation was to determine whether blood vessels could develop de novo between an extracardiac artery and a collateral-dependent zone of the heart and to quantify the nutritive blood flow afforded by the new vessels. We also adapted the preparation so that angiogenically active agents could be chronically administered directly to the site of neovascularization in subsequ...
